Write down what the person needs help doing: getting to appointments, managing paperwork, preparing meals, communicating with family or receiving professional nursing care. Ask the responsible clinician which tasks require a qualified healthcare professional.
Compare the proposed support
- Who provides each service and their relevant qualifications.
- Days and hours covered, including backup if the usual person is unavailable.
- Which tasks are included and what requires a separate arrangement.
- How changes in the person’s condition are escalated.
- How family members receive updates with the person’s consent.
Keep a shared care record
Identify the clinician responsible for ongoing medical decisions. Agree on how appointments, results and medication instructions are communicated. Ask the care team to explain the plan to the person receiving care as well as any authorized family contact.
For families living abroad, agree on a routine communication schedule and an emergency contact. A coordination service should state its actual availability rather than imply round-the-clock clinical monitoring.
