Keep an emergency information card
- Your current address, nearby landmark and accommodation contact.
- An emergency contact and preferred communication language.
- Medication and allergy information prepared with your clinician.
- Insurer assistance number, policy reference and payment contact.
- The treating team’s contact details if you are recovering from a procedure.
Plan around the location
Before a move or extended stay, ask which facility can handle urgent needs in that area and how to contact emergency services from your phone. Ask your treating clinician what symptoms require urgent attention in your particular circumstances. Do not rely on this general guide for triage.
After urgent care is arranged
When practical, ask a companion to notify the insurer and help collect records, receipts and discharge instructions. Clarify the follow-up plan with the treating team. For a transfer, ask who is arranging it and how medical information will accompany you.
Green Healthcare’s request forms are for non-emergency coordination. Discuss an emergency plan in advance if you need ongoing medical support during your stay.
