Patient Agreement

 

Patient / Client Agrees to terms and conditions below.

Green HealthcareCR is a private company where first-class medical services are rendered with providers throughout Costa Rica & Mexico. Green HealthcareCR is an entity dedicated to the reference of international patients around the world.

The agreement establishes that Medical & Dental and other services rendered in Costa Rica & in Mexico will take place at the quoted provider’s offices throughout said countries. 

In the event of unavailability of professional sickness or other. A reasonable attempt will be made, to assure that quoted procedure is performed with the same professionalism with a different facility with similar conditions. Always making sure it’s safe, and comfortable.

The care of patients at the facilities of agreed providers will take place within quote locations. The provider’s agreement with patients will ensure their care, support, and orientation with the purpose of offering them a personalized and pleasant experience during their stay and in the recovery sites which GreenHealthcareCR has quoted for patients. But in no way does GreenHealthcareCR own or operate facilities whose responsibility would be with said providers in the quote or service provider performing service .

 


 

GreenHealthcareCR will offer its patients trusted services internationally in the following Areas: Mexico & Costa Rica 

 


 

Green HealthcareCR References and will do the scheduling of medical appointments and consultations in coordination with the procedures.

The procedure costs budgets and the forwarding of quotes in accordance with that established by the Provider. And are agreed upon ahead of time prior to the procedure. Complications are not covered but are contemplated and an effort to avoid them with testing and prevention. In Mexico Complication Policy are available in the event of a need prior to the procedure.

Orientation for the patients interested in traveling to Costa Rica or Mexico will be done  prior  to all procedures at the provider’s Hospital through our contact center by telephone, email, or in person here at the Hospital.

Coordination of benefits with all the international insurance companies of those patients referred by Green HealthcareCr.

 


 

Green HealthcareCr  in some cases coordination lodging & transportation for the patients during their stay in Costa Rica & Mexico at Separate costs. “ not included unless all-inclusive package is selected

  1. Assistance for procedures with the admission department of the Hospital.
  2. Personal assistance such as escort to medical appointments and consultations
  3. Post-surgery or treatment follow-up and care of patients. by Provider.

 


 

NONEXISTENCE OF A LABOR RELATIONSHIP

 

By accepting these terms and conditions, there is no liability for services rendered by facilitator Green healthcarecr , no employment relationship of dependency, subordination and/or control between Green HealthCareCR SA and Patient, or between Green HealthCareCR SA and the staff, dependents or subordinates of  GreenHealthCareCR SA. Therefore, there is no right, interest or expectation of labor rights that may be argued by THE GreenHealthcareCR or its staff against the patient.

GreenHealthcareCR is for all legal effects of the Code of Commerce of Costa Rica & Mexico. Since the Patient has no liability towards the staff of Green HealthCareCR SA, it must handle directly with the management of  GreenHealthCareCr any matter related to the staff of this company.

Since there is no labor relationship between the Patient and the staff assigned to the service of  Green HealthCareCr SA, dismissals, substitutions, relocation, transfers and the disciplinary regime for the personnel who work for this company will be exclusively the competence of The provider. The latter engages to respect the legal and regulatory standards in the effect of labor law, complying with all the obligations, benefits, and responsibilities inherent to this kind of worker-employer relationship and maintaining its personnel duly insured.   Green HealthCareCR SA must include its staff in the contribution systems of the Caja Costarricense del Seguro Social and that is one of the Labor Risks of the Instituto Nacional de Seguros. The above must be shown through faithful copies of the contract issued by the Labor Risks Department of the INS, the monthly lists provided by this entity that details the names of the personnel covered by the insurance policy and the settlement receipts of the policies and worker-employer benefits.
