1. NON-MEDICAL ROLE OF GREEN HEALTHCARE

I understand and acknowledge that Green Healthcare Costa Rica and/or Green Healthcare International (“Green Healthcare”) is solely a healthcare coordination, administrative support, patient advocacy, travel assistance, and facilitation company.

I expressly acknowledge that Green Healthcare:

  • Is NOT a hospital, clinic, physician group, or emergency medical provider.
  • Does NOT practice medicine.
  • Does NOT provide medical advice, diagnoses, prescriptions, or treatment.
  • Does NOT control medical decision-making.
  • Does NOT guarantee outcomes, recovery, success, or satisfaction.
  • Does NOT employ or supervise independent physicians or hospitals unless expressly stated in writing.

All medical care is provided exclusively by independent licensed third-party medical professionals and facilities.

2. INDEPENDENT THIRD-PARTY PROVIDERS

I understand and agree that all doctors, surgeons, anesthesiologists, dentists, clinics, hospitals, pharmacies, laboratories, hotels, transportation companies, recovery centers, and related providers are independent third parties.

Green Healthcare is not responsible for:

  • Medical decisions.
  • Misdiagnosis.
  • Delays.
  • Surgical complications.
  • Infections.
  • Medication reactions.
  • Dental failures.
  • Implant failures.
  • Anesthesia complications.
  • Death.
  • Disability.
  • Dissatisfaction with treatment.
  • Actions or omissions of third-party providers.

3. ASSUMPTION OF ALL RISKS

I knowingly and voluntarily assume ALL risks associated with:

  • Medical travel.
  • Dental treatment.

  • Infections.
  • Delayed healing.

4. FULL RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by law, I voluntarily release, waive, discharge, and forever hold harmless Green Healthcare Costa Rica, Green Healthcare International, their owners, officers, directors, employees, contractors, coordinators, affiliates, representatives, successors, and assigns from ANY AND ALL claims, demands, damages, liabilities, losses, expenses, actions, causes of action, lawsuits, or proceedings of any kind whatsoever, whether known or unknown, suspected or unforeseen, arising directly or indirectly from:

  • Medical treatment.
  • Dental treatment.
  • Surgery.
  • Medical recommendations.
  • Travel coordination.
  • Transportation.
  • Lodging.
  • Communication delays.
  • Medical complications.
  • Third-party provider actions or omissions.
  • Injury, disability, emotional distress, financial loss, or death.

5. NO ADMISSION OF LIABILITY

I understand and agree that any communication, coordination assistance, scheduling support, refund discussion, accommodation, goodwill gesture, or continued communication by Green Healthcare shall NOT constitute:

  • An admission of fault.
  • Admission of negligence.
  • Admission of malpractice.
  • Admission of wrongdoing.
  • Admission of liability.
  • Acceptance of legal responsibility.

6. PATIENT RESPONSIBILITY

I acknowledge that I alone am responsible for:

  • Selecting whether to proceed with treatment.
  • Obtaining second medical opinions.
  • Understanding risks and alternatives.
  • Providing truthful medical history.
  • Following physician instructions.
  • Seeking emergency care when necessary.
  • Obtaining insurance coverage if desired.

7. NO WARRANTIES OR GUARANTEES

I understand that Green Healthcare makes NO warranties or guarantees regarding:

  • Medical outcomes.
  • Cosmetic appearance.
  • Recovery time.
  • Longevity of implants or restorations.
  • Pain levels.
  • Future health conditions.
  • Insurance reimbursement.
  • Eligibility for treatment.

All estimates and discussions are preliminary and subject to change after physician evaluation.

8. RIGHT TO REFUSE OR TERMINATE SERVICES

Green Healthcare reserves the absolute right to refuse, suspend, or terminate coordination services at any time for any lawful reason.

9. GOVERNING LAW

This agreement shall be governed under the laws of the Republic of Costa Rica Or Mexico.

10. VOLUNTARY EXECUTION

U certify that:

  • I have carefully read this document.
  • I fully understand its contents.
  • I have had the opportunity to seek independent legal advice.
  • I sign voluntarily and without coercion.
  • I understand that by signing this document, I am waiving substantial legal rights.

11. TRAVEL SAFE PORTAL DISCLAIMER

I understand and acknowledge that any platform, portal, application, website, QR system, communication tool, AI system, virtual assistant, emergency coordination system, telehealth interface, traveler registration system, or related technology including but not limited to the “Travel Safe Portal” provided, operated, referenced, or coordinated by Green Healthcare Costa Rica and/or Green Healthcare International is offered solely for informational, coordination, administrative, communication, or facilitation purposes.

I expressly acknowledge and agree that:

  • The Travel Safe Portal does NOT constitute medical care, emergency response, diagnosis, treatment, physician supervision, or guaranteed healthcare access.
  • Use of the Travel Safe Portal is voluntary and at my own risk.
  • Green Healthcare Costa Rica and Green Healthcare International make NO representations, warranties, guarantees, promises, or assurances, whether express or implied, regarding:
    • Availability of medical services.
    • Emergency response times.
    • Medical outcomes.
    • Accuracy of information.
    • System uptime.

    • Hospital acceptance.

To the fullest extent permitted by law, all services, systems, portals, software, coordination tools, and communications are provided “AS IS” and “AS AVAILABLE,” without warranties of any kind, whether express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, non-infringement, accuracy, reliability, or continuous availability.

I further agree that Green Healthcare Costa Rica, Green Healthcare International, their owners, directors, affiliates, contractors, representatives, technology partners, and coordinators shall have NO LIABILITY whatsoever arising from:

  • Delays in communication.
  • Technology failures.
  • Cybersecurity incidents.

I acknowledge that the Travel Safe Portal and related systems are supplemental coordination tools only and are NOT substitutes for direct emergency services, physician evaluation, or licensed medical care.
