Submitting this form securely sends an approval request to the Green Healthcare team. Approval is not automatic. Patient access is limited to patients assigned through the approved workflow.

1. Who needs access?

Select the role that determines identity, licensing and permission review.

2. Patient access and care relationship

Every approved patient can have a CRUM health record. Choose short-term or long-term care and every applicable program.

3. Health professional or organization role

Professionals see only assigned patients and the approved functions required for their role.

4. Government and Medicina Mixta role

Government access requires institutional authorization and is restricted to the approved public-program function.
